A dental treatment is an important decision so we want to make sure you understand exactly what is being done, the benefits, and the risks. Prior to proceeding with any treatment, fees and financial arrangements will be discussed with you and all your questions will be answered.
Beaumont Dental Centre accepts cash, personal cheque, debit, Visa, Master Card and Amex. payment is expected at the time the services are performed. Our office does direct bill to insurance and accepts assignments.

Filling out the form ahead of time will help make your visits to Beaumont Dental Centre as quick and convenient as possible. If you have any questions when filling out the form our staff will happily assist you with any questions on the day of your appointment.
